In order to better organize forms that come in to our staff, forms will need to be described in specific categories and named accordingly. The descriptive names are classified as DESCRIPTIVE_TEXT.

This text is user-defined information that assists the ministry purpose of establishing where the response came from. This might be the path to the page, it might be an article name, it might be a devotional date/title, etc. This text is what will be seen in the subject line of the receiver's email.

For the details below, refer to the following example: thelife(space)salvation(space)lifestory

• Information is to appear in a hidden field named "subject" that is either the last or second last field on a form.
• There *MUST* be a space between each required item. Spaces allow programmatic treatment of the field (or allows the program to read the items from left to right). These spaces are shown in the example above.

Response Categories
Each site that uses forms has a three-character site designator code (one of CWT, CHW, PTC, LIF, STL, TMM, WTM, CMT, etc). There are six response categories that our sites have (one of Salvation, Rededication, Prayer, Ask, OnlineStudy, General). No other response categories are permitted. In the example above, "salvation" is the response category for that particular form. It will always be the second word.

Detailed Breakdown of a Category
If a more detailed breakdown of a specific category is required (ie. General Testimony, General Author, General Editor, etc), the secondary definition or category must be the first word of DESCRIPTIVE_TEXT followed by a space. In the example, if there needed to be a breakdown it would look like "thelife_generaltestimony_lifestory". This breakdown allows for a more granular definition and is only for categories that require it.
